What should I bring with me when I come for a scheduled office appointment?

  • Driver's license or state-issued ID
  • Insurance card(s)
  • Copies of medical records, including x-rays, MRIs and CT scan reports, from prior doctor visits and/or hospital visits
  • CD of any previous imaging studies
  • List of all medications, to include: name, dosage, frequency
  • If your insurance requires a co-pay/deductible, please be prepared to pay that at the time of your visit. 
  • If your insurance requires a referral, we must have the referral prior to scheduling of your appointment.
  • It is helpful to write down a list of questions for your provider so that we can ensure that we answer all of your questions in the exam room.

Are my medical records kept private and confidential?

Your medical file is handled with the utmost respect for your privacy. Our staff are bound by strict confidentiality requirements as a condition of employment regarding your medical records. Ordinarily we will not release the contents of your medical file without your consent

What insurance plans do you accept?

Our providers participate in most major insurance plans.  Our appointment coordinators can review insurance information with you when scheduling your appointment.

What should I expect during first visit?

During your initial visit, you will be asked to complete a comprehensive health questionnaire. You will meet privately with a provider to review your past medical history, answers on your health questionnaire, and the reason for your visit. You will be able to speak directly to the health care provider and be examined. If necessary, prescription medicines, X-rays, laboratory testing, non-pharmacologic recommendations, and referrals to other specialists will be provided. If needed, follow-up visits will be arranged prior to leaving the office. Once test results are available, you will be able to visit with the provider and review the results.

How do I get a refill on my prescription?

We ask that you contact your pharmacy before your medication runs out to request a refill. Physicians may have to review your medical record before prescribing or refilling any medications. Please note that any new medications may require a patient visit and all refills require a visit every six months. Any new prescriptions or refills can take up to 72-hours to complete. Please always be sure to call your pharmacy before your medication runs out.

What if I need paperwork filled out by my physician?

Forms are dropped off at the front desk. Depending on the type of form, like FMLA, Disability and Life Insurance, it may take up to 5 days to be completed. Some paperwork might require an appointment with your physician for completion. 

Are there any medications I should avoid prior to allergy testing?

Yes, some medications interfere with allergy skin testing and should therefore be avoided prior to your allergy testing. This includes primarily antihistamines which are found in many over-the-counter cough/cold/sinus medications, oral allergy medications, allergy nasal spray and allergy eye drops. These medications should be held from 7 days before the appointment and until after the allergy testing is complete.
